Total cost for complete course is $2,999.00 FEES. Tuition and Foot Care Supplies ————– CAN $ 2,999 COURSE FEES: Breakdown of Course Fee Basic and Advanced Nursing Diabetic Foot Care Tuition Fee—————–CAN $2,350 Foot Care Supplies incl Starter Kit ———————–CAN $649 Includes: 1. A bag of one (1) set of foot care tools/instruments (Nail clipper 5’5, Ingrown nipper 1/4 jaw, Black File, Stainless steel Nail file and 10 abrasive pads). 2. Physical Book on Course Content Note: Student/s will be using the foot care tools during clinical practice. Nursing Scrub must be worn all the time during clinical practice CERTIFICATE If students require a paper copy of Certificate, it will be ready to pick-up from our office approximately 1-2 weeks after the completion date of the program. Students will be notified by email when it is ready. TERMS AND CONDITIONS 1. This agreement and attached schedules constitute the entire agreement between UHH and the student and supersedes all prior agreements, representations, warranties, statements, promises, information, arrangements and understanding, weather oral or written, express or implied, with respect to the subject matter here. 2. The student agrees to the following terms and conditions: a. The student will provide UHH with all necessary information as requested in its registration form and personal data sheet in use at the time of signing this agreement and b. The student has read, understood, agreed and signed a CODE OF STUDENT CONDUCT POLICY pertaining to the academic regulations of UHH which governs this agreement. A. The student agrees not to impart, sell, or give information obtained in the e-learning, practicum field-placement to reproduce or allow reproduction of the material in any form. 3. UHH and the Student each represent and warrant that there are no restrictions, agreements or limitations on their rights or ability to enter and perform the terms and conditions of this agreement. 4. This agreement shall govern in accordance with the laws of the Province of Ontario and the Laws of Canada applicable therein. CODE OF STUDENT CONDUCT POLICY The purpose of this code of Student Conduct is to highlight some of the key standards of behavior expected for students, specially regarding with clients/patients. Student/s are expected to deal courteously and tactfully with clients/patients, classmates, course director, family members, health care givers and staff during Clinical. Use of inappropriate or abusive language will not be tolerated and will be subject for dismissal/termination from the program. I. Protection of Confidential Information 1. As part of the program, the personal information and health condition of clients that student learns, will not be shared by the student with anyone else outside this program. 2. Privacy of photo and video, UHH seeks to avoid capturing images of people not being targeted for some specific, legitimate purposes. Student/s will seek permission of course director, clients/patients first, along with signed Consent Form, photography included or part of the treatment. 3. With respect to e-learning, or any computerized equipment and software, the following principles apply; a. Privacy and confidentiality must be strictly always adhered to. b. Any electronic services should only be “student” access, not for sharing, selling, posting to social media etc. c. Shall protect and maintain confidentiality such as confidential information including, manuals, Policies and Procedures, codes, copyrights, ideas, techniques, and know-how. II. Dress Code and Grooming Policy Student/s are expected and must maintain appropriate hygiene standards, including showering regularly and utilizing reasonable hygiene products such as deodorant. For females, fingernails should be clean and conservatively clean. Hair must convey a clean, professional, and natural look. For females, ensure that hair is up and not touching the face. Opened toes shoes, sandals and flipflops are not permitted. ONLY UHH scrubs are acceptable clothing during Clinical Practice, and free from wrinkles. Minimize the heavy use of make-up and jewelry, to look professional to clients/patients during the procedure and must abide for Health and Safety purposes. UHH strictly adheres to a scent-free/ odour free environment, including hair spray, perfume and deodorant which can trigger reactions such as Respiratory distress, headaches, migraine etc. Students are expected to exercise punctuality. If a student fails to report on a scheduled Clinical Training, communicate with the course director for any alternative make-up days. Advise course director of being late or absent immediately.III. Conflict of Interest A. Conflict of Interest defines when an individual’s personal interest or relationship interferes, or appears to interfere, with UHH interests. Students shall avoid any relationship or activity that might impair, or even appear to impair, when providing services. The student agrees that for a period of twelve (12) months following the registration in this program, the student and any Assistants will not solicit the clients , patients or healthcare facility working with UHH for student business relationship. B. Students shall not use UHH property or information for personal gain, or any opportunity that arises in the program. There will be fine of $50,000.00 (Canadian), when found guilty. C. Students who are found in a Conflict of interest will be subject for early dismissal/termination of the program without a refund. There will be fine of $5,000.00 (Canadian), when found guilty. IV. Dealing with Third Party “Third Party” consider client/patients, Agency of the field-placement, staff, students, family members, health care giver, colleagues, must ALWAYS practice privacy and confidentiality, student shall not; a. Disclose personal health information to anyone, unless to the POA (Power of Attorney), and Interdisciplinary Team members that dealt with client/patient health conditions. b. No students shall accept any gift of gratuity from any Third Party, unless during Holiday such as Christmas but not will exceed to amount of $50.00. c. UHH does not support any developing intimate relationship to any of the Third Party. d. Each student is expected to conduct themselves with integrity and in an ethical and professional manner while interacting with any of the Third Party, as per CNO.V. Commitment to Harassment, Discrimination and Violence-Free Environment UHH does not tolerate harassment including sexual harassment. Student/s are responsible for maintaining an environment free from this type of behaviour. ‘Harassment’ means engaging in a course of vexatious comments or conduct against an individual that is ought to know or reasonably to be known to be unwelcome. UHH prohibits harassment or discrimination based on an individuals’ race, ancestry place of origin, colour, ethnic origin, citizenship, creed, sex, sexual orientation, gender identity, age, recorded of offences, marital status, family status or disability, or other characteristics protected by applicable laws.Violence and threatening behaviour are NOT permitted under any circumstance, and all acts or threats of violence or the appearance of violence will be taken seriously. Retaliation against any Third Party will not be tolerated. The code of student conduct is a formal statement of UHH, intended for compliance by all student/s. Any student/s who have contravened the Code requirements, including failure to report any knowledge of wrongdoing, a conflict of interest, shall be subject to dismissal/termination from the program. ACKNOWLEDGEMENT I have received, read, understand, and will comply with the Student Code of Conduct Policy. I have no knowledge of any violation of this Policy. I understand that violation of the Code may result in the dismissal/termination from the program.
